Privacy Policy & Service Terms:

The following information is provided so that you, the client, may know what to expect from Surrey Helping Hands and that there is no misunderstanding. Due to the nature of Massage Therapy these guidelines have been set up for the protection of the client as well as the therapist.
1. A client intake form must be completed on your first visit.  This enables the therapist to check your medical background to determine if you are a candidate for massage.  Please be honest and thorough in your answers so we can determine if any contraindications exist.
2. We ask that you be on time for your appointment.  If you are late, the amount of time that you are late will be deducted from your massage hour.  This is necessary, due to the fact that there may be clients coming in after you and it would not be fair to make them wait.
3. If you find you need to miss your appointment, please give us as much advance notice as possible since other people may be waiting for an available appointment.
4. Draping will be done for comfort and modesty.  Only the area being worked on will be undraped.  If there is any area you do not wish to be worked, please let the therapist know.
5. If you expose yourself inappropriately to the therapist or if there is any unethical contact with the therapist, the massage will be discontinued immediately – full payment for the hour allotted will still be due.
6. Soothing music will be played during the massage.  If for any reason it is offensive to you, please let the therapist know so that a change can be made.
7. If at any time you should fall asleep during the massage, we will gently wake you when you need to be turned or at the end of the massage.  Otherwise, pleasant dreams!
8. All fees are paid at the time of the treatment and can be in the form of cash or check.  Any insurance billing will need to be set up prior to treatment and if the claim is denied for any reason, the client is responsible for payment.  We will be happy to provide paperwork necessary for you to pursue reimbursement.

Privacy Of Your Records:

The Federal government has passed a new law dealing with the privacy of your records.  One of the laws is that we get signatures from our clients on some forms so that we can continue to treat you or start to treat you.
As the government requires for this office, we need you to sign a consent form.  It tells you that we may have to share your health information with members of our staff to provide you with the appropriate care for your needs.
